Both are possible as well. If you set "Use file for configuration" CSVI will take the fields from your import file. If you want to add some fields with default data you can set the option "Add extra fields" to Yes. CSVI will then combine the fields from your template to the end and import those as well.

Does this mean that I don't have to set template fields, it just adds info to the database to the right columns?

Adding template fields is required for any template, CSVI has to know which all fields you are trying to import and if the record in import has to be inserted or updated which is done with help of template fields. For example, in Virtuemart product import product_sku field is identifier field, based on this field CSVI will process a record.

Yes, CSVI will look at the column headers which must match template fields and does the import. The order doesn't matter if you set 'Use file for configuration' option to Yes in Template settings on file tab but the name of the field should match. If you have your own custom headers in import file, you can specify them in 'Your field' while creating the template fields.
