It should be enough to import the product_sku and slug field if you want to import your own slug values. If you want CSVI to create them, you only need to import the product_sku and in your template set the option to re-create the slug to Yes.

You need to have two templates fields in your import template, product_sku and slug and that should help you change the slug field for a product. But If the header name in your import file is different than the template fields name then set Use file for configuration option to No in your template settings and on File tab. With this option, CSVI will read your import file columns in the order of the template fields added.
