It seems this is an error that can happen on PHP versions lower than 5.5. Attached is a patch file, please let me know if that works for you. You can load the patch file via the Maintenance menu.

You have a problem with the database table csvi_availablefields. Easiest is to delete this table using PhpMyAdmin for example, then uninstall CSVI, install CSVI and install the CSVI Addon that you need. You should have all the fields again.
